I grant permission for the above-named
participant to engage in all activities of the above named camp/workshop.
I assume all risks, hazards and costs incidental to such participation
and I understand that Robinson Drama Boosters, Fairfax County
Public Schools and employees of the above will not be held liable
for any injury, damage or loss suffered during the camp/workshop.
I hereby give permission for emergency medical treatment to be
given in the event I cannot be reached. I also authorize camp/workshop
staff to dispense above-named medication to my child. I will download and fill out the FCPS Emergency Care Information form found here and will bring it with my child on the first day of camp. I give permission for any camp/workshop pictures taken of my child participating in camp activities to be used for promotional purposes for Robinson Drama Camps.
